MICHIGAN MEDICAL MARIJUANA PROGRAM FOR PATIENTS

Many patients suffering from chronic pain find temporary relief with medical marijuana. That’s why we offer assistance for our patients with Michigan Medical Marijuana Program (MMMP) application certifications,  when indicated, via telemedicine or office visit. YOU MUST BE AT LEAST 18 YEARS OF AGE TO APPLY.

To start your online MMMP physician certification,  please book and pay for your visit using the booking calendar below.

Once payment is complete, you will receive a confirmation email with all the information you will need for your visit (via telemedicine or here in our Dearborn office).

Book and pay  $75 for your physician visit (telemedicine or in-office at your option).

  1. After your visit is confirmed, you can create your account with the state registry here. To reference Dr. Bazzi on your application form, please use physician record number 4301105966.

Step 3

Please allow 21 days from your physician visit to have your information forwarded to the state.
